For urgent assessments, referrers are welcome to complete an online Referrer Booking Form or to discuss cases directly with Dr Curtis.

First time patients are also required to complete a Patient Registration Form which is available for download. Please return this form via post, fax or email upon completion.

On-site drop-off facility and 2x patient parking spots available (No 103 & 104). Access carpark via Greenwich Rd. Additional parking on nearby streets: 2 hr free on Bellevue Ave or metered along the Pacific Hwy. St Leonards train station is a 5-10 min walk away.

There is free unlimited car parking in the Wilson Road Carpark, next to the Terrigal Bowling Club.

Access to Level 2 Commercial Suites is via the lift located near the pedestrian ramp/stairs on the Wilson Rd Carpark side of building.

When entering the lift, please use the Commercial lift access panel immediately to your left to access Level 2 Commercial. The panel on your far right accesses Residential floors only and requires a security card.

What to Bring

• Your referral letter
• All of your imaging (films or CDs) / reports / tests
• Your patient registration form
• A support person is welcome

The receptionist can advise you of the cost of a standard consultation at the time of booking. A partial rebate can be obtained from Medicare following the consultation.

We ask your understanding if there are delays or we need to reschedule your appointment at short notice, as Dr Curtis may be called to the hospital at any time for an emergency and occasionally complex procedures and consultations can take longer than anticipated.

Workers Comp & Third Party

Prior to your appointment, please obtain written approval from your insurer. We can assist in facilitating your approval, if we are provided with your full name, claim number, insurer and case manager details. However if approval is not received prior to your appointment, you will be responsible for payment of the consultation.

Out of Area Patients

Dr Curtis frequently services out of area patients, including international and interstate patients. Please inform our staff if you will be travelling long distances, so that we can maximise the efficiency of your visits. In some cases, an initial phone interview with Dr Curtis may be warranted, particularly for international patients. For near-by accommodation details, please click here.
